Understanding When Root Canal Treatment Is Needed

You might not experience much discomfort initially–perhaps, it is a mild pain sensation that comes and goes when you eat or drink hot or cold foods and beverages. Many patients think, “I’ll wait and see if this continues.” The sooner you come to see us, the sooner your discomfort will be ended and the more you increase the opportunity for treatment to be as conservative as possible.

There are multiple reasons why a tooth may become sensitive or become constantly painful. If the pain you feel is because tooth roots are infected or damaged, then removal of the roots, cleansing of the root canals, and filling of the root canals will be necessary to eliminate pain and preserve the tooth. When they recommend root canal therapy followed by a crown, it is because they know it will restore full comfort, functional strength, and naturally aesthetic appearance of the tooth — and predictably preserve the tooth for a long time.

Why is crown restoration needed after root canal treatment?

When the top of a tooth is opened to perform root canal treatment, a simple filling will not adequately restore strength to the tooth. It may temporarily provide support, but within a few weeks, the entire tooth above the gumline should be covered by a dental crown (sometimes called a cap). When a tooth is crowned, its form and function are restored for eating and speaking comfort, and for a pleasing smile.
When a tooth is significantly damaged or has been previously crowned, the core of the tooth structure may need to be built up to support a crown restoration. A core build-up is a common procedure but not always necessary. This procedure will not be part of your treatment unless it is necessary to predictably restore the tooth for a longer period.

When might extraction of the tooth be the preferred?

Some people are so fearful of root canal treatment and in such dental pain that they say, “Just pull the tooth, Doc.” We assure you that if we can preserve the tooth for years to come, the most conservative treatment is going to be removal of the roots and restoration of the tooth with a crown. We do these procedures every day for patients. You will be completely numb and pain free during the procedures. If you want to be medicated for relaxation during dental procedures, speak to us about our sedation options.

Patients typically want to save their natural teeth as long as possible, but sometimes removal of a tooth is the best option. If the tooth is too fragile to support a crown, Dr. Nilash Patel or Dr. Nitash Patel can remove the tooth and then provide crown and bridge treatment or implant treatment to fill the gap left in your bite and smile. Your individual needs and what will benefit you most will be considered and fully explained.
